History of CDPHP
CDPHP was founded by Capital District physicians in 1984 as a not-for-profit individual practice association (IPA) model health maintenance organization (HMO) in Albany, N.Y.

CDPHP and its affiliates have grown to serve nearly 400,000 people in 29 counties throughout New York state, including:

CDPHP continues to be a physician-directed plan today. A 15-member board of directors, including eight physicians, governs CDPHP, along with community directors who play a vital role in policy setting and administration.

The CDPHP practitioner network includes more than 9,000 providers and practitioners under four business lines:
- Capital District Physicians' Health Plan, Inc.® - HMO, Healthy New York, Medicare Choice, Medicaid, Child Health Plus, and Family Health Plus.
- CDPHP Universal Benefits, Inc.® (CDPHP UBI) - PPO, High Deductible PPO, EPO, and POS products.
- Capital District Physicians' Healthcare Network, Inc. (CDPHN) - Administrative services organization (ASO) and self-insured plans.
- APA Partners, Inc. - third party administration (TPA).
