health care practitioners, institutional health care providers and its pharmacy benefits

purpose other than for the administration, servicing or underwriting of a contract, claim or
account, or other purposes permitted by law without a members prior written authorization.
CDPHP does not share, sell, or rent any personally identifiable information about current or
former members.
The Information CDPHP Collects
You should be aware that when you visit CDPHPs website, it collects certain information
