CDPHP website. Information is the foundation of CDPHP's ability to provide quality service
and it is most important that CDPHP has your confidence and trust. CDPHP has gone to great

CDPHP uses safeguards and techniques designed to secure its member’s personal information.

member information and its contracted business partners, which may include CDPHP network
health care practitioners, institutional health care providers and its pharmacy benefits
manager to have access to that information.
CDPHP does not share member personally identifiable health or financial information for any
purpose other than for the administration, servicing or underwriting of a contract, claim or
account, or other purposes permitted by law without a member’s prior written authorization.
CDPHP does not share, sell, or rent any personally identifiable information about current or
former members.
The Information CDPHP Collects
You should be aware that when you visit CDPHP’s website, it collects certain information
that does not identify you personally, but provides it with “usage data,” such as the number
of website visitors or the pages visited most often. This usage data helps CDPHP to analyze
and improve the functionality of its website.
